Track order state progression

In order to disentangle the status of the order from that of the invoice, we have separated this information into two separate fields. This makes it easier to identify, for example, an order that has been invoiced but not yet delivered (order # 80) or an order that has been delivered but not paid (order # 79).

If invoicing is disabled in the account, then only the order state will show.
